TYPES AND FORMS OF DOCTORS EXCUSE NOTES

Yes, doctor’s excuses and doctors notes are the same and they come in different types and forms.

Many people expect a doctors excuse note to come with some kind of “Seal” or “Logo” but they don’t because a doctors excuse notes come from a physician and NOT a state agency or medical society. Even if you get a doctors excuse note after a stay in the hospital, the certification is from the physician.

A doctor’s excuse note is actually a legal document and not a random note excusing you from work or school etc. There are (4) four elements every doctor’s excuse note should contain:

1. The doctors contact information, including name, address, and phone number.
2. The date the note was generated.
3. The name of the patient or subject involved.
4. Certification of medical consultation

Doctors excuse notes can take 3 main forms:

1. “Letter Head” – A note form the doctor that includes the four elements above on letterhead stationary from the doctor’s office.
2. “Prescription Pad” – Some doctors use a regular prescription pad that they use for medications. This form is popular because it is quick and convenient especially if the reason (certification) for the excuse is short and simple.
3. “Special Form” – Some offices and many hospitals have a special “fill in the blank” form labeled and specifically used for medical excuses.

Most actual doctors excuse notes and the best fake doctors excuse notes come in the Letterhead form.
